Fill Out Our Referral Form

Please contact Jon Freer by phone or email to enquire if there is available space for the program you are interested in.

For individuals you would like to refer to our program at Behavior Wizards, please complete the following steps:

Step 1: Click on the "Download Form" button. The referral form will open in a new window.

Step 2: Save the referral form to your computer and rename the PDF by including the person’s first initial and last name that you are referring in the file name (For example: BehaviorWizards-ReferralForm-MJones.pdf).

Step 3: Please complete form.
